They might not admin that that is their reason, but once you say "no" to a company, its going to be harder to get a "yes", in the future.

Unless you're under contract with guaranteed promotions, you can be "denied" a promotion for anything. Your employer is under NO requirement to give you a promotion. They aren't even required to give you a job tomorrow!!!!

It depends largely on how the denial or promotion would affect the person making the decision. If it would put a star in his crown to promote you or if it would hurt him. But, yes a denial can be made.

If they do not want to promote you they will not, you are either over qualified or under qualified for the position.
over means threat to their job in the future
under means you think you are more capable than what you are.

Sure, so long as that reason is not pretextual for a discriminatory purpose (i.e. not giving you the promotion based on your sex, age, race, religion, etc.) and so long as they're not violating any collective bargaining / seniority agreement.
