It is very ill. Is its body actually kinked? So many goldfish are churned out, and so many are inbred, that it is common to find goldfish with kinked bodies. If this is the case, chances of survival are slim.

However, your fish sounds sick, not deformed. What do you keep him in? The #1 cause of illness/death among pet goldfish is improper care. Goldfish are poop machines and they produce high quantities of toxic ammonia. If they are not kept in large enough tanks with proper filtration and oxygenation, they will not live very long.
A fancy goldfish needs 20 gallons of water. Other types like koi, commons, comets, need very very large tanks like 100gallons, or prerably ponds. They are BIG fish, and can grow to 12-18".

sounds like he has what is called swim bladder disease it is treatable , but first i would take in a water sample to your local pet store most of them will test your water for free , just to make sure you havent had an amonia spike that can also cause the fish not to eat , b/c pretty much the water is funky and they feel like crap , also start using aquarium salt 1 teaspoon per ten gallons your store should also sell it , even though goldfish are considered freshwater they have found that using the salt will act as they the fish is taking a daily vitamin and help prevent against fungal and bacterial infections , the medicine that treats the swim bladder is called fungus eliminator made by the company jungle , it will turn your water yellow for the time you are treating the tank

do a water change and stuff! make sure its not too hot!
