this place sucks and i wanna go to another doctor

U can do that, just get them to make u a copy of your medical records. It is the law that u can get a copy of your medical records.
Or , u can just go to another doctor and that doctor will have them transfer your records by computer.
Simple stuff.

Your health records are your own and no physician can refuse to release them to you. This is protected under the Health Information Privacy and Portability Act or HIPPA which went into effect several year ago.

That being said, you cannot simply take your chart out of your doctors office because that record does belong to his office and he may need it for future reference in terms of your care. What you can do is fill out a request for release of medical information in which case copies will be made of the records and given to you or sent to any new physician you authorize them to be sent to. Likewise, if you want another physician to have access to those records, you fill out a release form and their office can contact your current physician's office and have the records mailed or faxed. Technically, they are your records. However, MOST medical offices will not let you walk out with them. They will forward them to the doctor that you tell them to. The thing is, although they are your records, they office has physical "custody" of them. They aren't usually willing to just hand them over to you.

You can not take them, but you can request them to send copies to another office or make copies for you. You probably need to sign a release to get copies done though.

no you can't but you have to sign a waiver , for your new doctor to request them , then they have to send them via courier to the new doctors office , but after 5 years you can request a copy for yourself , and they have to provide you with them , that is law

you'll have to go into the dr's office file a release form and then thay will fax them or maybe thay will let you take them w/you.

patients can review their records, but depending on your physician- it may be a violation of HIPPA to copy them
