I have a 99 grand am and it has nothing but water in the radiator. I went to start it this morning and let it run for 20min, when I went to leave the temp gauge was in the red. Could this be caused by just having water in the radiator? Would a flush and refill wil antifreeze fix the problem? Or could it be something worse?

Your fan should have came on to cool it down. Check to see if the temperature sensor is working, the fan relay is good, and the fuses for the cooling system fans are OK. The fan or fans should come on when it gets hot like that, even if you have no water in it.
The thing to do is make sure you put a new thermostat in it, and fill the radiator with a 50/50 mix of the coolant that is called for in your owners manual. Check the system by pumping it up with a coolant pump and looking for leaks, and that it holds pressure. Replace the radiator cap with a new one that is the correct pound cap for your vehicle. Make sure you bleed the air out of the system, and you should be back on the road in no time. Hope you haven't hurt the engine by letting it over heat.

You should at least have a 50/50 mix of antifreeze water conducts heat more then antifreeze. Also check the thermostat also once I had a car that kept overheating no matter what I did and I found out the previous owner put a bunch of radiator stop leak in it and over time it plugged up and I had to get a new radiator so you may want to bring it to a good automachanic to check for obstructions.

Check your oil
if oil OK then good chance that thermostat is stuck
water will not cause engine to overheat as long as there is enough but put antifreeze/corrosion inhibitor in as soon as possible

oh dude not good thats the problem with those cars i had a 97 and it did the same thing u probable messed up the head gaskets or theres a leek

water wont do that.you need to flush it out and change the thermostat.also check to see if the fan works and if those things dont help,then it could be the water pump.

u proabably ruined the engine friend. if it wasn't frozen ur thermostat stuck closed and it overheated, and most likely cracked the head.
