not sure what the term is for where a river begins. But the mouth of a river is where it flows out. Rivers usually start as small mountain streams or are just inland water that was created by the ocean.

The place where a river rises is it's source and its mouth is where it enters the sea or other large body of water.
A delta is the alluvial plain around a river mouth forming a triangle of land and water.

It is called the "head" or the "source" of the river. The mouth is where the river empties.
