It depends on what you mean by 'underrated.'
If you mean, what sign gets the least attention astrology-wise, I'd refer to a sign like Capricorn. They aren't the type who usually jump into a conversation to tell everyone their star sign to begin with, and the profiles you find on them don't seem very sensational, because Capricorns are simply steady, level-headed people.
Cancer also gets ignored, because people are too busy making jokes about cancer as a disease, the fact that the symbol is a crab, and that the glyph looks like a pair of boobs to really get into what a Cancer's personality is like.
If you mean, what sign tends to get a bad rap, then I'd definitely say Scorpio. Lots of people have this perception that Scorpios are all these intense, sexual people who wear black leather and walk around starign at everyone intensely. And while this is true in a large number of cases, it highly discredits the members of the sign that are not like that.
If you mean, which sign is often thought to be a softie but actually isn't... Then I think I'd refer to Libra. People have an image of them as these people who are fabulous and host great parties and wear pretty clothes, but have no substance to them, when the fact is that Libras are also known for their intelligence.
People also think that all Leos are self-centered, and even where this is true, Leos are also very caring people.
But I could go on and on about the misconceptions people have based on astrological signs.

Pisces are the most intuitive sign. They spend a lot of time trying to help people by relating to them. They can effectively put themselves in others shoes, and consequently feel their pain. It can really take it's toll. They are definately the mosr under-rated sign.

The most unfortunate answer is Scorpio. Why? I can not find the answer. People are charmed and attracted to Scorpion, till they do not find out the sign. I am sure, they are prejudiced. You can not deny intelligence, creativity, devotion, love and passion of a Scorpion.
