does anyone know what a nutritional care model is?

and i dont mean a model like a person, its this thing that i have to write a report on. it was formed by the health services research in 1998 and all i know its called a nutrion care model. thanx an answers would be great!

anybody that is good with healthy living and interested in nutrition can surely help?

Usually in a case like ths model would mean the ideal or the perfect standard to reach for. This would be the perfect balance of vitamins, minerals and nutrients to become or stay as healthy as possible. I know thats a pretty generic answer, but without knowing the actual agency who came up with the actual model, its the best I can do.
