If it isn't a big deal, then I wouldn't bother.
If this is a huge problem, that'll cause you to get caught or whatever, then yeah.

Its always best to admit a mistake, especially in the workplace wher it might impact others. Your boss will probably be really mad, but will also probably respect you for taking responsibility. Look at the mistake you have made and look at things you can do to lessen the impact of it. If you go in thre with an apology and a game plan set in place it won't be so bad. It'll be worse if your mistake gets found out as then you'll look stupid and so may your boss if he is asked to explain it by another senior member and if he doesn't know you've made a mistake then he'll look stupid and he won't thank you for that! Plus you will feel better for getting it off your chest. You don't want to be looking over your shoulder at work 24/7. Take some courage and just do it. Good Luck!!

Most employers don't like surprises. If the mistake was "huge" it will be found out eventually. Talk to your boss about how the mistake was made, why it was made, possible solutions to correct it and how you learned from the experience.
If he/she has been in management for a while, they know that with responsibilty given to employees, mistakes do happen. Think about this - If you were the boss and this happened to your employee would you feel better if they came to you to discuss the issue with you or if you found out the error on your own?
Trust and credibility are the key.

If you have made a huge mistake, you have to ask yourself what the results will be of the mistake. If no one will die as a result of it then maybe it can be lived with. If it will cost a huge amount of money to fix you are probably screwed. What are the chances that someone else will discover it? If someone else discovers it and tells your boss before you do you are screwed. Whatever you do, don't lie about it, this will just get you in worse trouble. I think that you'd better just tell your boss one on one, in private.

Let your boss find out by normal means and accept total accountability when he does. It would also help to look extremely surprised when he shows you the scale and scope of your mistake.
On the other hand, DON"T HIDE YOUR MISTAKE or OBSTRUCT others from finding it.
In Corporate America, you get no bonus points for being the bearer of bad news.
And confessions may relieve you of your your guilt, but it may also relieve you of your job.

go to your boss and tell him what you have done along with impact and solution ,
Be ready to take responsibility for the mistake.

Honesty is always the best policy! Your boss is more likely to be forgiving and let you keep your job if he finds out from you and not from something else.
You will run the risk of being fired, depending on what you did, but he will probably find out what you did and it will be better if it came from you
