The kiss at the end of II erased her memory. The original ending (because I and II were shot at the same time) was the spinning back of the world angle used in I, to make time go backward and her to never know. They decided to use it for 1 because they didn't really have an ending. Donner expected to have time to come up with a new ending for II, but he wasn't brought back for it.

At the end of "Superman II", he kissed her when he was in her office. While they were kissing, he somehow hypnotized her into forgetting his identity.
Yes, she remembered it in "Superman IV: The Quest for Peace", but he made her forget again.
However, since the events that led up to "Superman Returns" pretty much stomps on the other two films that came after "Superman II", we can pretty much take it as "gospel" that Lois has been made to forget Clark and Superman are the same.
(And besides which, I ain't buying that whole thing about the kid is his anyway.)

In 2, at the end he kisses her and she forgets all of the events in the film. if you recall, Lois asked a fellow staffer what's been going on in the world, and the woman looked as though she'd lost her mind.
In that era of Superman Mythos some sources give him psychic abilities so that is the source basis for the mind wipe kiss.
As to the not knowing his ID, I think that it looks bad that Superman knocked her up w/o allowing her to keep that know ledge. but also the idea of his secret ID is that since he is a public figure as Superman and makes no attempts to disguise himself, like Batman, that he is Superman 24/7 and does not possess any ID beyond Superman, world's biggest Boy Scout.
In Returns, you may also notice that both Richard, and Jason looked at him as though they might figure it out, and as Richard started to put it together Clark heard him, and he tripped to make him think twice. I personally think Jason has figured it out, but being 5 no one will listen if he tells them.

Because she's an idiot, like every one else who can't figure out that a guy that looks exactly like superman and never wears a mask and is always disappearing when superman shows up might just be superman. Especially if you conceived a child with him, how could you be that close and still be that clueless?

Lois Lane. Mary Jane were given no sh*t hostile to Lois. Lois gained a Pulitzer, Mary Jane did not. Lois were given a superchild, Mary Jane doesn't have one. because she's sterile. So, i say, LOIS LANE.

honestly, i found myself wondering throughout the whole movie how he is able to disguise himself by taking off his glasses and slicking back his hair (except of course for that little curly q of hair), and it kinda makes ya question the intelligence of all the other people, ESPECIALLY Lois....i mean, they had sex, you'd think she would have been close enough to his face to go waiiiit.....i know you from somewhere!! but then, no person could end up with perfect hair and not a single scratch after being tossed around a crashing plane either, so you just kinda have to accept that it's a movie and that's just the way it is.
