Most of the ones where I live have the 1st window aka "bubble" as its called in fast food, closed with paint on the windows. The bubble is used when its busy usually at lunch and dinner times for a few hours only . Its so one cashier can just focus on drive thur order takin and collecting money without having to make drinks or deal with handing out bags of food. It is called Bubble because it is usaully off in its own lil corner in the store usually in a back storeroom or in its own ill room in the back.
The ones that permanetnly keep the bubble closed is because the bubble is too close to the drive thru speaker where you order, so it just makes it worse and backs up cars by having both windows open, so they just use the 2nd window and forget the 1st one.
Take a look at your fast food places that always keep 1st window closed, Its too close to the order speaker.

not if theres a window number 3, then windows 1 and 3 will get used, and number 2 is used to showcase the bun racks.

It's not just McDonald's, but other fast-food restaurants as well. Sometimes, they take the money at the first and pickup is at the second. Maybe that's a little more efficient.

Actually, window number two is used most often. It's window Number one that is only used occasioally.
