That's a very hard one. There have been many great movie villains, so to pick one that is the greatest of all time is one heck of a challenge.
After some thought, I will have to go with the character of "Uncle Frank" from the movie "Hellraiser". While many think that the Cenobites are actually the villains, they would be incorrect. The pivotal characters in the film are the Cotton family, and the newly resurrected "Uncle Frank" Cotton.
Frank manipulates Julia, his brother's wife, into bringing him victims in order to sustain and strengthen him. Frank also kills Julia's husband, taking his skin in order to rid himself of Kirsty (the daughter) so he and Julia can be together.
He is manipulative, violent, and as the movie progresses the viewer cannot help but hate him, and feel repulsed by him.
When the viewer of the film feels so strongly about a villain, then that character is successful.
This is why he gets my vote.

Dr Evil, he brings a sense of class to the big screen while still maintaining the villainous edge.

Dracula, he gets the girl AND gets the girl. there can never be a super power villain that can match the count

Darth Vader does it for me because his first name implies "Dark" . How would you feel if someone could kill someone with just thought processes like he did? I know that sounds a bit contrived, but it's kind of freaky when you think about it.

Hannibal Lector. He was crazed and evil but there was also something compelling and sympathetic about him too. I didn't want him to be captured or imprisoned at all. He was darkly sexy as long as you weren't his next meal.

Count Olaf...Series of Unfortunate Events...I know it sounds stupid, but I really think that he takes the cake...especially if you read the books
