It all depends where you are going to put it. You really only need a clean-out on your kitchen sink, utility sink, water softener, and washer. As far as having them on your "lavs", they should all have a removable p-trap on them. Meaning that if you ever have to "snake" them, they can just unscrew the p-trap and clean your drain out that way. Hope that was of some help. Good luck.

Most residential p-traps disassemble easily so no cleanout is required. For commercial uses you should consider what is going to pass through this line. For example, a drinking fountain should only have water passing through it so a clog is unlikely. A kitchen sink can have all sorts of things that can cause clogs (grease, hair, food) pass through it so a clean out should be used.

Anytime you can install a cleanout the better. just think about getting a clog and not having a cleanout.

most homeowners don't require a clean-out at the p-trap, they are usually installed in commercial applications, but if you want to install one it could make you're life easier if ever there was a clog

depends on the chance of how often it will get cloged up. typically no clean out is nessansery. you can get a p trap for under your sink that comes all apart. it is pretty cheap
