mine was when i was passing a note to my friend saying "i have athletes foot" and the girl i was passing it to to pass to my friend read it and told everyone. i was embarrassed

I had rented this really cute townhouse with a girlfriend...it had this really neat loft area that looked over the living room and was really nice.
Anyways one night I was out with my boyfriend and we had a few too many to drink & ended up back at my place...my roommates car wasn't home & the lights were off so we knew she wasn't home...and well we got busy in the living room.
About 5 minutes into our playing we hear these giggles coming from the loft...my girlfriend, her bf, 2 of his friends and 3 of our mutual friends were up there watching us. They had been out and had come to crash at our place.
The embarrassing part was that we had been playing with some ice...let your imaginations run on this one. To this day I am still called "the Ice Queen" by these select people.

Way too many to count. I have at least one extremely embarrassing moment per month.
Most recent was probably when I went out with my friends and my wife stayed home and invited a friend over. They went on our computer to surf and they went into the history and found that I browsed to at least 50 pr0n sites. I cleared out my IE history, but there is a bug that doesn't always clear out your history. I have since changed to Firefox ofcourse :)
My wife was like, "I feel like I don't know you, etc. etc." and her friend told everyone in our circle of friends!

Umm probably in high school, I was wearing a skirt and I got out of my boyfriends car, put on my back pack and my skirt got caught under the back pack. Lucky for me I was wearing a thong so half of the school and my class got to see half of my ***. A girl that I didn't really know came up and told me about it. I was so embarrassed.

I was out on a double date and started feeling car sick. Someone in the back seat mentioned going for pizza I threw up in my boyfriends brand new car. sheesh!!

when i sat on stage in a little play and my legs fell alsleep ( i was little ) and i was trying to get up and i accidently pushed this boy and made him fall down off of the stage.
