One of their best friends is secretly in love with them (either the best friend or the partner) and conspires to break them up by creating doubts and misunderstandings.
or
A family crisis separates them and love becomes difficult during this separation. Perhaps if the family crisis takes one of them back to their home town, the first love could make a return to the scene.
or
One of them overhears a conversation between the beloved one and a friend and, not knowing the background, completely misinterprets what is said. He/she then become cool and distant which is misinterpreted by the other and the relationship suffers a crisis.
or
A serious illness
or
A new and very demanding job that brings one of them into contact with a whole new world of glamorous, jet setters and temptation and jealousy abounds.

1. The guy and his family are passionate Christians but the girl is a Muslim from Iraq(or Iran, etc.) who doesn't take her faith as seriously as the guy. They both meet in med school and gradually form a bond that ignites into passionate love and an inseparable fondness for one another.

2. The guy's family warmly receives this young woman but the gal is going back to war torn Bagdad during the semester break and wants this guy to accompany her and meet her family. However, her family does not warmup to this guy as readily as the guy's family.

3. Perhaps, while in Bagdad, the couple happen to travel down a "wrong" road and get ambushed by one of the radical factions like the Shiites or Suuni's(don't know how to spell these) and during the ambush the guy is taken hostage but they let the girl return to her family. Or, maybe they keep them both as suspected spies.

4. During this inncarceration period, let's say maybe the gal's family starts to soften a little even if out of sympathy for the guy and in seeing how much their daughter cares for this guy.

5. Let's say things do work out, not only with him being released safe and sound but also for a happy and healthy relationship between the two with the blessing of the girl's family. Perhaps they go on to get married while still in med school and later set up a family practice or even opt for seperate but compatible specialities that allow them to continue on in their medical careers while sharing their love for one another and their lives together.

His parents are immigrants and they are always pushing him to study and get good grades, and they want him to marry someone from their culture, and he knows that they think girls are just a distraction, and she's not from his culture.
She's religious and took a virginity vow, and he's not religious.
Her parents got divorced and now she's really cynical about love and marriage.
She's a goth girl and he's a jock, and their friends would poke fun and them if anyone knew.
He's good-looking, and she might be, but she has glasses and braces and a bad complexion and frizzy hair, and she pretends to not care about how she looks, but she does.
She's brainy and he's not.
She has soemthing wrong with her spine and is in a brace from neck to hips, and she can't do sports or anything and she might need an operation to fuse her spine together.
She's in a wheelchair with cerebral palsy and it's hard for her to talk.
He's deaf and she doesn't know sign language.

so many problems have been done to death to the point of cliche. Good luck breathing new life in!

Let's see:
-one's divorce was never finalized, and the spouse comes back to haunt them (this could tie into several different problems: trust, jealousy, cheating, court)
-a local bigshot has his eye on the lady, and is willing to go to great lengths to get her (has his rival beaten, files false charges against, threatens, bullies)
-one has a shady past to hide from the other

Interracial, where the guy likes the girl but she doesn't like him, but after a year the girls starts to like him, they go out for like 8 years and keep it on the dL cause of the interracial propblem then the girl cheats on the guy cause she falls out of love and the guy goes real crazy then either a good or bad ending. Let me know how it goes. Gotta have comedy, romance, sex, and must touch the heart. Its my story.

Cheating?
One of them has a friend who fancies the same person?
Lack of trust?
Scared of getting hurt?
They don't want to spoil a good friendship?
One of them's going to move away soon?
They're young and reallly in love but want more experience from other partners before marriage?
One of them wants kids and the other doesn't?
One of them doesn't believe in marriage the other wants to marry?

you should put them in a situation that they don't know what they have in common and are trying to get to know each other better.(or else they might break up)

a supernatural angle is sure to generate lots of problems... why not make one of them a werewolf, or psychic, or one of each....
