Ivillage has a program where you can do that and forms. Other than you can paper mail your stuff to a consultant.

Im not sure of an actual program...but if you were to search the internet, you would find a site!!

l'oreal used to do it for their hair colouring products but i haven't been on their site for a while - you could try tho
