I think they are fairly synonymous but if I had to choose I would say that modifying is smaller in scope. You can modify different assignmentsto allow for sifferent ability levels. Differentiating hasto do more with instructional practices as a whole -choosing different instructional techniques to account for different talents, interests, and abilities. That's my guess.
