I don't know about the college and high school levels, but I believe that the NFL steroid usage is quite low. The reason is that the players have decided to get rid of them. Back in the 70's when steroid use became an issue, the NFL was unique due to the fact that the players approached the NFL with the demand that steroid testing be done. The reason was that the majority of the players did not want to use steroids, but were afraid that they would have to if no testing was done.
The NFL tests for steroids every few weeks, and can demand a surprise sample at any time. Yet, year over year, you only hear of a case or two (if any) of suspensions due to substance abuse.
