I am bad at note taking and test taking a I need some suggestion on how to make my study and note taking better.

Talk to the Professor during Office Hours and discuss what can be done to make sure you do your best.
Always do the Reading Assignments and Homework.
Ask for a Tutor!
Always talk to the Professor when you have questions. They love to see people trying their hardest and making the effort to learn.
Also, ask the Teacher if they have any pre written notes for you to follow. Some teachers give study guides or lecture notes out from their webpage.

Use a voice recorder and take notes.

When you get home, play back what you have recorded and compare it with your notes. Just to make sure you didn't miss something.

Study, study, study!

Do study guides and set up an appointment with your instructor to go through the topic again.

make an appointment with a school counsellor and ask him or her about study skills seminars (lots of colleges have them), as well as tips and resources to help you out. Some colleges even offer a non-credit course designed to help the transition between high school (or years in the workforce if you're a mature student) and college.

Yours are common problems and easily overcome with a bit of help. Good luck!

Do the reading assignments, then when it is discussed you know what to ask. This way you can understand what you missed in the reading, scores points with the teacher too.
