it is possible, the penis is inverted and scrotal skin makes the lips. no you can not tell it was a man before by looking at their vagina. the urethra is moved inside so you can urinate testies are removed and so is the penis shaft the penile skin forms the vaginal canal.

Do you really want to know, they split the penis and turn it inside out to make the vagina and ensure use all the sensitive tissue so can make a clitoris and all. A friend who was married with kids left his family to be a woman, Peter became Louise after months of counselling and approval he became she, she had to use a dilator like a dildo and stick in the hole for so many hours a day so it was deep enough for sex until it moulded that way over time. She occasionally showed friends and with pubic hair it looked real of course being gay I was not going to get in there and test drive it. The hormones grow the breasts and feminine features but some men still look too big as women unless plastic surgery is done to make more feminine.

Yes that surgery is possible.Its refered to a gender reassignment surgery(GRS) or sexual affirmation surgery(SAS).
The procedure is performed by de skinning the shaft and removing the testicules.
The extra skin from the scrotum becomes an exrention to the skin tube that was on the outside of the penis.
The urithra is disected from the shaft and kept intact for the time.
The muscles that composed the shaft is amputated and a piece of the gland is sutured on top of it to form the clitoris.
A cavity is cut into the PC muscle to form the vaginal vauld.Then the skin tube is inserted into this vault and kept in place for 7 days to allow the skin to graft to the muscle.
The urethra is cut back to cover the clitoris and provide the urinary track opening.
the skin from around the incission is formed into the clitoral hood,inner lips and outter lips.
After 12 months you cant tell that this used to be a man.
The sources for this answer comes from my experience of having gone through this procedure in march of 2006.
I have had full medical exams for employment and had to inform the Doctor and OBGYN that i was not a genetic female.
Uppon looking at the genitals you could not know there was a penis there before

The surgery is real, and it is almost impossible to tell (you'd pretty much need to be a gynecologist performing an exam...)

The penis is not cut off, it's actually used in the process, so that the resulting vagina will have most or all of the sensitivity. The 'tubing' is sort of re-wired so that one can urinate just as a natural woman does.

1, most of the time men don't get breast implants, they take hormones that allow for natural breast growth. The same hormones a girl going through puberty has.

2, yes you can have surgery to get a vagina. And lips. Yes, all the vaginary things.

3, you'd have to have seen quite a few in your time. Chances are you wouldn't know. Surgery is very convincing.

4, testicles aren't required other than for sex, so they're removed. The tubing is rewired so you can urinate properly.

5, that's fine, caps.

6, I'm not going to go searching around for TS vaginas.
