I have studied Palmistry for 30 years, here is the definitive answer. The non-dominate hand indicates inherent traits one is born with. The dominant hand, that is the hand one writes with indicates what one has done the traits he or she was born with.

Sometimes you will meet someone who is ambidextrous, in this case, have them interlock fingers, as if one where praying. The thumb on top will be the dominate hand, regardless of the "handed-ness."
And yes most every part of the hand is read: skin, nails, even fingerprints.

There is no difference when reading men and women's hands.

both hands are read in palmistry not one. And further more, both sides, fingers and knuckles are read too.

One hand is your actual real life hand, the other is your spiritual life. If you are right-handed, that is the predictor of your real life, while the left is your spiritual side. If you are left-handed, the reverse is true.
