She was in on the movie, but she was apparently totally surprised at him trying to put a bag on her head.

She was in. Borat would rather surprise her, but after consideration, they have to let her know before. They couldn't get through the security and troops of armed body guards otherwise.

GQ magazine, Newsweek, Rollingstone and a few other magazines imply that she was in on the joke (GQ says she definitely was in on it)
