A few reasons. Firstly, In the case of a "Stay of execution" after the insertion of the needles, there shouldn't be any death or sickness caused by the procedure (Slim as the chance of lethal infection may be).
Secondly, it's a matter of safety to those doing the procedure. If they should accidentally be stabbed by the needle, at least it will be clean, so they won't catch anything (And they will use a new nedle on the "victim")
Lastly, and probably the biggest reason, The equipment used are all standard medical items. Usually, needles are sterilised straight after manufacture.

And as for the alcohol pad, the same dealy as the first point applies, but it often also makes the veins appear easier, helping facilitate insertion of the needle.

Needles come pre-wrapped and already sterilized. To use a dirty needle you would need to go find one.

They aren't going out and actually sterilizing a needle just for the execution, they come that way.

This is a much asked question

I believe it has quite a simple answer

The injection equipment comes
in a sterilized form as standard,

it will protect those who are administrating
the injection, from possible disease contamination
.

This of course does nothing ,
for the person to be executed . :((

Law and standard practice (OSHA) requires it. I can just see the lawsuit filed by a family member that they didn't prep the site and sterilize the needle. In today's arena of trial lawyers, they would probably win.
