In a orchestra there is the 'cream of the crop ' musician and the 'lesser' talented musicians. The 'better' violin (fiddle) player sits in what is called 'first chair', the rest of the violin players (fiddlers) sit in the following chairs. Playing 'second fiddle' is a phrase known to be ' second best ' . . . not the main person. In business or a relationship it would mean to be the kept to the side as second best, not the main or favored person. "Cream of the crop" would be " first fiddle ".

It means that you are not the most important person in someone's life. You come in second. It comes from the seating in an orchestra. First violin is the coveted chair. Only the best violinist gets chosen for that chair. Everyone else is "second fiddle".
