What's the correct size, should the designer show all the work he/she has done, is there some web source about this topic, thanks for the help.

Letter size or a little larger is good. Don't want it to be too big or bulky. 12-20 images is good, best stuff first and try to keep them in some sort of order. Making a cd or dvd slide show presentation is also nice. Make sure to have your contact information and some resumes with it if doing it for employment.
They sell portfolios at places like hobby lobby or art supply stores. Make sure the plastic cover to the pages is very clear. You can also have a book bound yourself at staples or some place like that.

I'm not sure about web sources I will check on it if noone responds.

use ms frontPage. it's have redy templates to create own website who don't know html
