How much does each of the items listed cost

I'm assuming you're asking about the US; here are the fees:

Copyright -- $45 per application

Trademark -- $325 per International Class. All goods or services are categorized within International classes. Goods run from classes IC 1-34, while Services are in IC 35-45. You can check that out here: http://tess2.uspto.gov/netahtml/tidm.html

NOTE: The USPTO does offer a lower cost option of $275 per IC. However, the application must be near perfect when filed -- any changes that have to be made incur additional charges

Patent -- There are a number of fees at the various stages. Here's a listing of all of them:

Copyright is secured automatically when the work is created, and a work is "created" when it is fixed in a “copy or a phonorecord for the first time.” For example, a song can be fixed in sheet music or on a CD, or both. Although registration with the Copyright Office is not required to secure protection, it is highly recommended
