Manufacturer recommendation is 105,000 miles or 7 years under normal conditions, 60,000 miles or 4 years under severe conditions. Severe conditions are defined as "a vehicle driven in conditions over 110 degree F or below -20 degrees F. This is what is called a "high nitrile belt". They last longer than the older type.

Ideally just before it breaks. Unfortunately you can't tell by looking. So what the manufacturer does is gather statistics on that particular repair, car A the belt broke at X miles, car B...etc.
They add all those miles together and find an average and from that extrapolate a "Belt is likely to break at mileage A, so if it were replaced X amount before mileage A, that would be good."
Then they put that in the book. So read the book and believe then when they tell you it's a good idea to replace.

You would normally replace the Honda timing belt every 50 to 60,000 miles. If your car misfires, is misaligned or starts missing you need to have it checked out.

Every 60K miles. You can stretch it but if the belt breaks, you'll be faced with the more expensive repair of replacing bent valves. Honda's run very tight tolerances that if changed (i.e., broken timing belt) will force the valves into the tops of the pistons.
