Pixel by pixel. The image on TV is just a bitmap that changes 30 times per second (sometimes more, sometimes less, depending on the video). The TV scans from left to right, top to bottom (I won't get into progressive vs interlaced) and draws the pictures one by one. It can do this so quickly that to you it looks like a moving picture.
