can you give us some more info? What kind of fish is it? What size tank is it in? What are its tankmates? How often do you clean the tank?
The white stuff, is it fuzzy? If so, that is a fungus, and you should treat with Maracyn-2.
Is it white spots, like sand sort of? If so, that is Ich, a parasite. Put 1 tbsp of AQUARIUM salt per each gallon of want water, disolved in a cup of dechlorinator water, in your tank. Over the course of two weeks, it will kill the Ich parasites. Don't use Ich Meds from the petstore - that stuff may just as easily kill your fish.

It may be too late but you need to treat the tank with ick medicine. You can buy at Walmart, or pet store and it just takes drops at a time depending on how big tank is, also I would pick up fungal treatment, it will be in same area. The black could be fungal. Do you have something to feep water moving filter etc.

Sounds like ick. If you go to the pet store and get some ick treatment, you should be able to save it. You may also want to descibe what is going on with your fish to the people at the pet store so they can help you find the right medicine.
