When can a company have "Chief" positions (CEO, CFO, CTO)?

What are the "qualifications" - size, revenue, time in business, etc? If you could provide a reference (link) that would be awesome!

No requirements except being incorporated and allowing it in the corporate structure.

There are no qualifications, except the corporate form of business organization. Moreover, the key word is not "chief", it's "officer". Legally speaking, only corporations have officers; the term is legally meaningless in any other form of business organization...
