Not necessarily. The actions of sex can just cause more to be force out than if left to nature.

As long as you have adequate foreplay, you shouldn't bleed much during sex (but you and your partner should both be ready for a little bit of blood). There's also a lot of evidence that sex on your period can reduce menstrual cramps!
