The bank will provide the forms. They're the ones under the legal obligation to give you the forms.

It depends on how your doing the deposit.
I would recommend a wire transfer for that large amount of money. If it is a check I would recommend two forms of ID and a contact number for some with the issuer's bank who can verify that the check is legitimate. If someone was to bring that check to me I'd want to make sure that the check was real and not a joke.
Another recommendation is call ahead to the bank that your taking the check to to find out any other special instructions they have.
Also know that the funds are NOT going to be made available the same day unless its a wire transfer.

The bank is required by law to records deposits over $10,000. If this is a legitimate insurance settlement, you have nothing to worry about. If you are in any doubt about the source of the funds, don't spend it for a week or so. If the check is bogus, the bank will want the money back. Grandpa

Go to Swiss bank, they never asked anything....as long as the cash comes in, they'll be just fine..

the bank will take care of any forms,, as you will be a preferred customer,, any depoist over $10000.00 is reported to the goverment,,, and you will have to prove where you got this money... guarntee
