Nope. A lot of schools do require it, but some don't. I don't think it has any bearing on whether it's a good school or not. You will have to read about the program you are interested in to see if they require it. Also, there is a general GRE that many schools require, but there is also a subject GRE (ie. chemistry, biology, psychology, etc.) that some schools also require. Chances are, unless you have your heart set on one particular program, that you are applying to a number of different schools, and at least one of them will require one or both exams. It doesn't hurt to take it. If you don't do well, you don't have to send your score to a school that doesn't require it. Also, even if a school does require it, they may not put as much weight on it as you may think. They also look at things such as your undergraduate GPA and if you've done any work related to your field already (for example, in the sciences, have you completed and/or published on any research as an undergraduate?).

No. Some private schools don't require it, but some do. Most public state schools do. The score you need will depend on the school and program you want to enter.

No, Sometimes other tests are used or accepted for admissions. You should definitely check the specific programs you are looking into to see what is required

Nope -- just the good ones.
Of course, if you are going to a professional school (for JD, MBA, MD) then you take a different exam.
