I have heard of it happening but I'm not sure

This is very rare, but yes it can happen. A few weeks after a man has the procedure done, he needs to provide an ejaculate sample for testing to ensure there are no sperm making their way out of the vas deferens. This is a little embarrassing, time consuming, etc. and not all men follow up and have this done. This is the only way to catch the men whose healing process doesn't totally occlude the vas deferens and sperm (few) are still in the ejaculate.

If executed correctly no he can't get a lady pregnant. The whole intent in a vasectomy is to preclude pregnancy. He desires to go in for semen evaluation to ensure his sperm level is at 0. A vasectomy will also be reversed. However the man can have a low sperm count and the couple would wish fertility treatments to conceive (i've a pal who went through this).

after a vasectomy a man needs to go for a post vasectomy semen analysis. If his analysis comes back ok...ie no semen...he can't get a woman pregnant.

NOWAY, he is sterile and there is no way it could happen.
Make sure the man really had it done.

no they tie off the tube that producess sperm but it can be reversed
