I think it is not likely he has 3, perhaps a collection of fluid called a hydrocele, or a spermatocele, these can feel like a mass distinct from the real teste. If you shin a flashlight thru the scrotal sac, the real testicle will not let the light to shine thru, but the collection of fliuid, like a hydrocele, will light up. I would try this one at home before marching off to the urologist.

if he's a man why hasn't a routine physical or Doctor come across this situation? If he hasn't seen a Doctor for a physical ever then it's time he uses some type of insurance coverage available and have himself get checked out. This 3rd testicle thing should be the checked out by a Doctor, not by the people on Yahoo Answers!

Obviously you shouldnt ask if its normal because up until now I thought ALL guys had two. Has he been to a doctor? I hope you find out what it is, he may have shared the whom with a twin who died and gave him a testicle or maybe he just has a growth.

I don't know how common it is but I have heard of things like this. Just talk to him as embarrassing as it may be. He is probably the best person to tell you. He's had it all his life I assume.
