Something she can enjoy now-
Beautiful bouqet of flowers, candy, or a perfume she likes....maybe some homemade bread etc.
I'm guessing she would really just like anything- I don't know a lot of 89 year olds that haven't only wanted their family around.

How about a coupon from you redeemable for 2 hours of uninterrupted time with her at her convenience.
2 hours she can enjoy with her grand child any way she pleases. (baking cookies or reading the Bible together or better yet BOTH)
She will love it and do not be surprised to see a tear or two form in her eyes.
