What goes on the top line doesn't concern them as much. If you know which person is on the lease, put their name first:
Jack Tripper & Chrissy Snow & Family

There should be rules for this as it happens so often! When I was living with my son's dad people just addressed things to us using his last name. I didn't mind. But, before my brother and his wife were married they liked things addressed to both of them by their first and last names. You could also try To the Smith/Jones family. Or something like that. But, really it depends on how well you know then and what you think they prefer. If in doubt you can always ask them.

Address the envelope as Mr Smith and family (or Ms. Smith)
Use everyones first name inside the card

Address the envelope to the person that you are closer with (if you knew the guy first, address it to him). In the card, use their first names only.

By their names, of course.
On the envelope:
His First Name Last Name
Her First Name Last Name Name
Child's First Name Last Name (if desired)
Their address
In the card:
Dear Her Name, His Name, Child's Name
It's easy. Always address people by their names, it works like a charm.

On the envelope, address it to both of them using their full names. Inside, use everyone's first name!
