Try something to do with kids - kids are sick at school, you have to go pick him/her up. If you don't have kids, try nieces and/or nephews - say the school can't get ahold of their mom and they called you to pick them up. Most bosses understand that, especially if they have kids themselves. If your boss is a male, try using the excuse of cramps or "female problems." In those cases, most male bosses will not ask for details.

Is this personal experience that he won't let anyone leave early? You might be surprised, I was when I asked my boss if I could leave early when we were at our busiest time of the day. She asked why and I told her that I just didn't feel like being there. You could have knocked me over when she said yes!

Try the direct approach first. If that doesn't work, you can always go to him at a later day with a fictitious appointment that would enable you to leave early - of course, you'd have to let him know in advance about the "appointment".

You had the last appointment with your dentist for the day. He called and had a cancellation, asking you to come in earlier. If you can't, you'll have to reschedule your appointment because he won't wait for you.

That's a tough one.

Start moaning, run to the bathroom with your hand over your mouth, splash a bunch of water in your face, hold your breath to turn real red.

Go to his office and tell him you just hurled, and need to go home.

Good luck.

Tell him your irritable bowel syndrome is acting up. He won't want you to explain.
