My grandma is 93 and each year I either get her a subscription to the newspaper (she still loves to read) or a gift certificate to the hair salon (however that is probably more than $20). The newspaper can be done in small incruments, such as weekly or monthly for very little cost. Some other ideas are her favorite food or candy, a book or a book on tape if she is not able to see very well, a warm throw blanket, slippers, or a pretty plant. I hope these ideas help.

If she's able to travel, you could take her out for lunch... A change of scenery may be quite welcome, even for a little bit. Someplace with a nice view maybe?
Large print book or large print Readers digest subscription. (If she's still able to read) If she's not. You could get book on CD or tape (make sure she has a player) or buy a regular book, and make regular appointments to read to her.
Some company is likely the best gift you can give, so if you don't follow any of the above suggestions, I would suggest you work in some time with her into whatever you decide on.
Bless you. You sound very sweet.

Well, unless that 90 year old lady has been working out lately, it probably wouldn't cause any lasting damage, or even hurt, if she punched you in the face. I would probably be like, "What?" And walk away unless she answers. But of course there aren't too many old ladies throwing punches out there...

Ladies of age really prefer the 'pretty, finer' things in life. A pretty silk scarve or a lovely throw would sufice. Are you family? Take pictures of her children when they were young and make something out of them. (Duplicate them1st.) Ladies of age really prefer sentimetality.... they love remembering 'way back when'. Find a nice pair of pearl earings if she has pierced ears (or clip ons) . (You can get faux ones for around 20) At this point in her life...she really just wants someone to spend quality time with who will listen to her stories and do things with her.
Good luck!
