I think the most important things in a good fantasy novel, as well as any genre are strong characters and a compelling plot. All the other things are just dressing because if you have a fabulous world but nothing else, it could alienate your reader.

I like the freedom method. You go through life, as if you weren't going to write at all, and then, when you least expect it, WHAM. You're in a fit of inspiration and you find yourself dropping everything to pick up the pen. And that's it, no planning, no nothing. Just writing. Best plot ideas come this way ..Or there's the fight for it method. Get some time to go somewhere you love and write. Not as easy as it sounds, and you may or may not write, but very rewarding....other than that, I don't plan anything. Sometimes I brainstorm with my friend, who's an artist, and write it down, but that's it. I think all those other plotting ideas stifle creativity. They're too structured. You just can't control this kind of thing, you know, its inside you.
