Physically in simplest terms:
An antenna is a piece of metal that if you put an oscillating voltage on will produce an electric field. The cell phone tower is another piece of metal that encounters the electric field and measures it. Using some math that voltage put on the cell phone antenna is made to resemble your voice. Hence transfer of your call.

A cell phone is a small two way radio. It communicates with cell towers. Complex algorithms and an administrative set of communication protocols with the towers to determine when the cell phone changes towers and frequencies.
