Just in case you don't get an answer from someone who has actually done it correctly, I'll describe my best shot.
You have a chart, which is a circle divided into twelve segments. You start by placing the sun on the chart. In effect, the chart has you at the center -- that is, the location of your birth -- and all the rest of the objects, sun, moon and planets, are positioned relative to where they were when you were born. Now, the sun sign is easy to place; you mostly already know your sun sign. It's the segment of the sky the sun was in when you were born.
But the moon and the rest of the planets were there, too. So you find where the moon was by the time you were born. Put that in the sky (on the paper). Then there are tables in a book called an ephemeris (although I'm sure today there are computer programs instead) which can tell you where the planets were on the day you were born. Put all those on the chart.
Now you look for patterns in how things line up on the chart. There's clues in whether the planets are clustered to one side or scattered, in whether they form squares or triangles, and so on. You also can correlate the four classical elements to the signs, and therefore look at whether you have planets in a lot of water signs (emotions; feminine magic) or whatever. How those various clues are interpreted is, of course, the matter for years of intense study. Again, it would not be surprising if there are computer programs that simplify it considerably.

The majority of them are just made up. Some are done through complex astronomical measurements and following trends in astral bodies, but most are just people making it up.
