I would suggest any additional accomplishments including ones that don't have anything to do with the subject matter of the book.

Rather than including hobbies and birth date I would write about your education, your writing experience, activities related to that, like if you have done / participate writing workshops, seminars etc especially if a well known person if your field participated too. Include the title of any other book that you have published or if you have written articles for a magazine or news paper. Also, you can mention that at the moment you are working on your next novel or researching for your next book and mention the topic.
I hope this help.

You have a book ready to go to print and the publisher or editor hasn't revealed what they exactly want to go in the back of the book?

Don't give a complete life history, but keep it capsulated, brief. Have your biography focus around the book being published.

Is this something that the editor gave you for the cover or that the publicist gave you? What kind of book is it? Who is the intended reader, and what facets of your life are likely to resonate with him/her?

You need to talk to your editor or to the publicist assigned to your book about the marketing strategy that they have planned. You should tell them about any contacts that you have that might be useful, and discuss what you, yourself, might be able to do to support their plans.

After you have that conversation, you can write a focused paragraph, where every word is chosen to pique the interest of the reader most likely to buy your book.

One of the things that you should do, is to find the celebrated reclusive American writer and poet, Linda J Cirillo and pick her brains about the way forward into the realms of publishing.
