Real pearls, when rubbed against your teeth will have a gritty feel to them. Fake pearls will have a smooth feel.

Drop them in water. Plastic floats, real pearls sink. (For this purpose, 'real' includes cultured peals with a ceramic core. If you get your pearls from some cheap bastard who cultures with plastic cores, they'll also float.)

Bite them. If they are real they will feel gritty. Fake ones will be "too perfect" and smooth.
