If you are talking about qualifying for your next belt the percentages vary. As your belt level gets higher the required health percentage also rises. These percentages are approximate - they haven't been narrowed to the exact number in every case. I can't guarantee that that low end numbers are the 'bottom level' in every case.

Working on the white belt: 12% and under - not qualified, 16% and over - qualified.
Working on the Yellow Belt: 21% and under - not qualified, 28% and over - qualified
Working in the Orange Belt: 40% and under - not qualified, 43% and over - qualified

For the following belts I have exact percentages:
To qualify for the Green Belt - 56% or higher
To qualify for the Blue Belt - 70% or higher
To qualify for the Brown belt - 84% or higher
To qualify for the Black belt - 98% or higher
