when you watch tv programs or movies with subtitles, write down the words you don't understand and look them up in the dictionary afterwards. in my case, i have this notebook where i write all the words i don't understand.
if you also come across words that you do understand but don't normally use, write them down, including the whole sentence if possible. then try to make your own sentences using those words. you will be amazed that your vocabulary will expand in such a short time.
also, the English you learn from movies and tv shows is the kind spoken everyday by the English speaking people. this is very different from the ones you read in books. my suggestion is that you study grammar and you do everything you can to learn from movies and tv shows. this will provide you with a good balance. you won't sound too bookish and you won't also sound too informal.

Hey friend there's a lot you can do to improve your English and I appreciate your spirit and willingness to improve the most.
1. Enrol for a English speaking course (pretty obvious one)
2. Read good books to improve your vocabulary.
3. Do not hesitate to ask others the meaning of something you dont know.
4. Else look it up in the dictionary or google it. This will give you lot of information on proper pronounciation, usage, synonyms, etc.
5. Speak English whenever and wherever you can. There's nothing like practice to improve fluency.
7. Ask friends/family to correct you whenever you go wrong.
6. Make it a habit to learn one new word every day. Learn its meaning and draft multiple sentences using that particular word.

Watch English language films and tv programmes (if they also have English subtitles it makes it easier).
Listen to English language radio (BBC Radio is available through broadband internet connections).
Read books (graded readers if you aren't yet confident enough to read them in the original), newspapers and magazines (you can find a lot online).
Listen to songs and try to understand the words, you can usually find the lyrics online.
Try games and puzzles - word searches, crossword puzzles etc.
Meet English speaking people and practice speaking and listening.

Try watching DVD movies, and turn on "English Subtitles". That will help you learn to read, because you can both hear and read at the same time. And it's a fun way too!

Just keep reading and writing, watching TV in English and radio too. Then you will to find English speakers to talk to so that you are fluent.

Tune to BBC & CNN. Read novels and pay attention perticularly to the conversation part. You will pick up fast. Most important is to find some one with whom you can always converse in English. Talk talk talk.

Try reading childrens books, they have simple stories and small words. Also listen to English music to know how to pronounciate words better.
