Go to the court in question and ask the clerk for the filing papers; if you are POLITE and NICE they will help you.

Pay the fee and get a court date.

If you have BEEN CLEAN for 5 years it will generally be a slam dunk.

File a case directly in court for the expungement of your records citing that it was committed a long time ago and you have not committed an offense afterwhich.
