In Nevada, they are listed by the first letter of their last name for each office, thus alphabetically. There are states that do it differently, but I believe that some people would find other methods confusing.

Depends on the jurisdiction - where i live they assign ballot positions randomly

I think it is pretty standard for the listings to be alphabetical.
