well there are several factors that could affect titration accuracy.

1) unclean apparatus while using titration

2) washing the flask with the solution that is going to be titrated. it will cause your readings to be more.

3) parallex error while reading off values

4) too quick to add the solution in the burette to the flask and hence missing the end point

5) lack of stirring of the flask
