i got a book report due and im suppose to have a title page. the paper said this:

the report must include a title page with the following: name, date period, title of book, author, and # of pages in book.

what am i suppose to do i dont get how to write that as a title page. please help or give examples please. thank you

Titles are the names listed at the library the name that
is on the card in the box to look up the book's location.
Here is the sample: say A BOOK of poetry by emily ding.
You have the book, where does it go. To look the book up
in the card catalog, or on computer file, the title is ""Book of poetry"" author is Emily Ding, and # pages, whatever it is.
The number of pages allows you to count the preffice if you
want. Say it is 190 written on pages of poems, 3 pictures,
3 pages of preface, and one bibliography. that is then 197. You
do not count blanks, separators of silk, or cardboard and so on.

Get a blank sheet of paper and put it all in order:

My Name
12/1/06 - 12/5/06
"The Count of Monte Cristo"
by Alexandre Dumas
lots of pages

Titles get centered in the middle of the page. You instructor has told you what he or she wants.

I could give you an idea, but who knows if it's what your teacher wants. Ask your teacher to be more specific. It's the only way you'll get it right.
