As for the family, just be completely honest with them when talking about your personal life, including your desire to change sexes. With your girlfriend, you need to tell her as soon as possible. Thats a damn shame, anyways I think you should just tell them. Be like I'm not happy with myself and would feel better if I was a female. Simply say that. I mean if you are over 18 you can do whatever the hell you want to do but if you are under 18 thats just to bad. And for your girlfriend just tell her straight up. Tell her unless you want to date a female this relationship is over. Whats the worst that can happen? Would your family love you anyless just because you like a boy?

Sit them down and explain to them that you've always felt different, and that you have never felt like a man. Tell them you've always felt you were in the wrong body and that you have always been a female inside instead of a male. Just let them know who you are, and whether or not you plan on going on with gender reassignment surgery.
